Amelia House Transitions Residential Service
About Us
Amelia House opened in January 2020 as a specialist transition service for young adults transitioning from children to adult services, people who are at risk of placement breakdown or placed out of area looking for a roadmap to greater independence through positive risk-taking, person-centred planning and holding the people we support at the centre of our work.
Amelia House and our community services has won two national awards at the Learning Disability and Autism Awards 2021 and 2022 and been a finalist

OUR ETHOS
At Amelia House, we believe that the thoughts and wishes of each individual that we support should be at the very centre of the care and support that they receive.
Each time that we welcome a new individual to start their journey with us, we create a unique, tailor-made package of care. We spend quality time with them and their family, to find out what their goals are, and how we can best help these to be achieved.
We believe that our services offer the perfect balance of care, support, guidance and compassion, and we are constantly striving to better the lives of each individual that we support.
OUR PURPOSE
Amelia House is a 10 bed residential service providing 24 hour care and support to the individuals who reside with us. We work in a caring, therapeutic and PBS informed approach. Over the years, Amelia’s purpose has changed and is now to support young individuals with autism, learning and physical disabilities. We have key workers and core teams that ensure our individuals are supported and have the best care.
DEVELOPING INDEPENDENCE
Our skilled staff help the people we support to engage in activities which help promote and upskill their independent living skills. The people we support will have a dedicated key worker and a plan developed with them and their family.
Within our houses, we also hold regular Independent Learning sessions for our individuals, around areas such as finances, budgeting, healthy relationships, interview skills, staying safe online, and much more!
We continue to support individuals back into the community to live more independently within our community services once we feel they no longer require high levels of support within Amelia House.
Amelia House, aims to promote each individuals independence, by encouraging choices.
PERSON CENTRED CARE
We ensure that our home is as person centred as possible and have an amazing maintenance team and staff team that create the most amazing murals for our young individuals. All ten individuals have their rooms personalised and have designed these themselves with the support from families and staff.
MY KEY MY LIFE
It is a 12-18 month programme to develop an individuals skills to successfully transition into their very own home via WDH or another housing company while supporting them with their recovery. We like to celebrate individuals’ success and achievements throughout your time at Hapton and really help you on your journey!
We provide a steppingstone service or an assessment-based environment to understand a person’s long-term care and support needs and also work hard to provide an independent learning roadmap, building community links, educational or employment opportunities or working closely with professionals to reduce incidents.
We have currently moved 15 individuals, from Amelia House, into our community services due our dedication to achieve the best for each individual.
